It is important to submit claims with the beneficiary's name and HIC number exactly as they appear on the Medicare card. If the name and HIC number on the claim do not match the Medicare card, the claim will deny as unprocessable. If you have a claim denied for this reason, notify the patient and confirm the information.
Advise
patients to contact the Social Security Administration for name changes or corrections.
Keep a copy of the beneficiary's Medicare card on file.
