Field 11

Field 11 is for the insured’s policy group or FECA number information. This field is a common cause of denials. You have two scenarios for completing this field.

Medicare is Primary: In most situations, Medicare is the patient’s primary insurance. In this situation, complete Field 11 as follows:

• Field 11 – Enter the word "None" in this field.
• Fields 11a, 11b, 11c, 11d - Leave these fields blank. Completing these fields will cause the claim to deny.

Medicare is Secondary: Some patients have another insurance company that pays before Medicare. When you submit a claim to another insurance company first, Medicare wants to know where you submitted the other claim. In this situation, complete all parts of Field 11 (Fields 11, 11a, 11b, 11c, and 11d) with the primary insurer’s information.
