Signature Requirements

All documentation and forms must contain the physician's signature.

• Documentation submitted on letterhead is acceptable without a signature if you are the only doctor in the office.
• In a joint practice, each doctor must sign his or her documentation.
• Electronic signatures are recognized and acceptable.
• Initial your transcribed notes.
