For documenting chiropractic services, Medicare does not prescribe specific forms. You may use the forms that work for you.

• Clearly address all the documentation requirements found in the policy.
• The best form is the one that is completely filled out and addresses all the points in the policy.

Refer to the Documentation module for more information. A set of example forms is available from the Florida Medicare website under Part B > Chiropractic Services. Note that these forms are intended for educational purposes only. You may use them if you find them helpful, but they do not constitute a prescribed format.
